You should get information before you travel so that vaccinations can be planned, ideally no later than:
Holidays – at least 8 weeks before you travel
Backpackers and working abroad – 3-6 months before you travel
We will need to know:
- Departure and return dates
- Countries visiting
- Type of stay (holiday, backpack, occupational etc)
- Allergies
- Female patients – pregnant now or wanting to be
- Vaccines you have had previously
Please note that there may be a charge for certain vaccinations and the vaccination required may not be available.
Please complete the attached questionnaire and bring back to the surgery at least 8 weeks prior to travelling to ensure the vaccination is available and an appointment can be arranged.
